need help with Swiftech H220 all-in-one watercooler, extreme hot temperatures.

About a month and a half ago i bought a new all in one CPU cooler, a swiftech H220 as i heard it was a good solution. As soon as i recieved my cooler i installed it into my case and decided to test it using realtemp and prime95, as a result the tempratures wouldn't go near 50 degrees celcius under 100% load.

Today i found out that my tempratures are going well over 90 degrees and my radiator isn't heating up anymore. So i decided to open up my case and have a look at what is wrong now i concluded that my heatsink feels warm, and i can almost squeeze both sides of the tubes together, so my guess would be either they are already empty or nearly empty

My question is how can it be that an all in one CPU cooler which is pre-filled at the factory can already be empty or nearly empty after a month and a half of use when the manual states it only needs to be refilled every 3 years depending on room temperatures which in my case usually stick between 20 to 25 degrees.

Could it be that my CPU cooler is faulty, could it be possible that my cooler leaked while i didn't notice anything about it, i have never seen moisture or vapor inside my case. What is recommended i do in my case?

A little update, i have just removed the cooling block, on the underside of the copper there are now heat marks from the CPU holder it didn't look quite good. I reapplied thermal paste the size of a rice grain and screwd it back proportionally, after i had done this i ran another test and my temperatures just went straight back to the 90 degrees. So i decided to check in my bios how my pump is doing, and apperantly it is now running on 8 to 900 rotations instead of the 5 to 600 when i first installed it.

For those wondering what the hardware is i'm using:
Case: Coolermaster HAF 932 (model 2008 not sure if there are newer ones) ventilation: 200mm side fan, 140mm back fan, front needs a new one, top 2x120mm with the H220 radiator, all fans are blowing into the case with the top ones blowing out.
Motherboard: Asus maximus VI Formula
CPU: i7-4770k @ 3.5Ghz (stock no overclocking done yet)
GPU: Asus Nvidia GTX690
