Annoying Video Problem

Hello guyz,
I am experiencing a weird video related problem. i will sum up the indications/facts and then i will expand my question. i have a fairly strong machine so none of the below problems should normally occur
- PC specs -

Asus P6T mobo
Intel(R) Core(TM) i7 CPU 920 @ 2.67GHz (bloomfield 45 nm)
2x HDDs (one dedicated to the Windows instalation, 500gb, one dedicated on games and Software, 640gb)
1x External Drive USB 2.0 Western Digital Essentials 2TB (for Movies and series)
Microsoft Windows 7 Ultimate
Ram DDR3 8GB 2100 MHz
NVIDIA GeForce GTS 450 ( 1024 MB)
Display Resolution 1920 x 1080

Problems:

- I play a game, not a very demanding one on terms of gpu load (Heroes of Newerth online game), and i got mini frame rate freezes and more often asynchronization between video and audio, for a few seconds. Some times, the whole screen freezes for like 10-15 seconds completely and then it resumes. I have noted that the intensity of the problem increases if i have a chrome browser open espesially if more than one tabs are open.
- i try to play a hi-res video (720p and higher), on numerous players including VLC, Windows Media player, and WMP Classic, and i got massive and continuous "lagging like" effects on my screen. The video jumps and its choppy and the audio acts accordingly.
- i try to watch hi-res videos from YouTube, happens the same thing as above.( i mentioned this as a 3rd problem/indication, even though it was the first thing i noticed, cause someone might assume its an internet connection based problem when clearly it isn't)
- skype video calls appear to have the same problems as the ones described above (less often, it might be internet connection based lag)

Actions Taken:

I have first noted this problem back in August, where it was summer and my machine showed some overheating issues. I addressed them, solved them yet the problem persisted. I updated nvidia drivers SEVERAL times, i even formatted the machine cause i thought that even though my security was high, i might have some how gotten my self some malicious software that was running on the background leeching of my PC's power. I downloaded some software like, What's Running Now to see if there was any extreme background activity, nothing. The workload of the cpu is always very low, everything looks normal, my pc is safe and optimized with latest drivers, nothing looks wrong yet i cant enjoy normal flow videos. I also completely changed the RAM and upgraded it from 4gb to 8gb, nothing changed. Note that the game i play, performs extremely well while playing it, perfect frame rate everything as it should be given that it is not a demanding game, BUT then the described problems occur and its very annoying.

Conclusion:

I think something is wrong either with my nvidia graphics card or its software OR - perhaps- and i dont know how to track this, some problem with my HDD? Meaning that the reading speed of the HDD has been reduced therefore the system lags? I don't know how to locate the problem please help.

It's not the heating problem, I already had asus pc probe, the temperatures i got since i fixed the problem was an average of 50 Celcius for CPU, <40 for Mobo. All fans are currently working and the dust levels are normal. I used the program u suggested it says thermal status is ok. I can provide screenshots if u would like to have some. Btw scout_03, amazing program, loved it already. So many details!
