Alright, so earlier today my GPU was working just fine and working as normal. Suddenly when I came back it started revving and making my desk vibrate. I felt the top of my computer and it was steadily fluctuating. Imagine if you repeatedly tapped on the gas petal on your car and the engine would make noise rhythmically with your foot. I opened MSI afterburner and it was up to a whopping 85C peak temperature. It didnt stay there long. And it eventually fell back down to around 40-50C. It goes back to doing this every 10-15 minutes without warning. I'm only running Steam, Google Chrome and Skype...Normal stuff and I've never had a problem. It just randomly picks up heat without warning and the revving intensifies once again.

I tried running antivirus and removed some threats, but it hasn't helped. Have I effectively fried my card?

CPU: AMD Phenom II X4 945
GPU: GTX 560
PSU: 420 Watts

when the GPU temp arise, is the GPU being used? check on MSI afterburner or gpu-z if there's any gpu load when the temp arises, if you've OCed your GPU try stock settings.

GTX560 should have lower idle temps than 40-50 (though that varies significantly depending on its cooler ofc),
