Irregular graphics crashes during game play

I'm having an issue where recently my computer has begun crashing at non-constant times during different game-play. At different points in time across different games, both of my monitors will suddenly display an image similar to this [http://i.imgur.com/jfzZkdC.jpg] . Sound will continue to play for a little while, but I will be unable to speak with or hear my friends via mumble. The only solution I've found to break out of this is to just shut-down / restart the computer via buttons on the tower.
I've looked around and it appears to be a hardware issue from what some forums have said, but I was wondering if anyone know what exactly is causing my problem and if there are any solutions. I'm also wondering what replacement card I should be looking at if I do have to replace it. Will I need to replace my motherboard and CPU as well? The computer was built in the spring of 2011, so is almost three years old. System specs are bellow (let me know if I missed something you need).
