Upgrading Computer Advice

Hello,
I'v decided to upgrade my computer for 2014 as my current rig now is 3 years old and was wondering If anyone could help me decide what upgrades would be recommended and which upgrades I should focus on

[Current Rig]
Cpu: AMD Phenom II X4 965 Processor x4 ~3400MHz
Graphics card: GeForce GTX 480 1500MHZ
Ram: Corsair 8 GB
Motherboard: ASUS M4A89GTD
Resolution:1920x1080

I'v generally got into gaming more recently and would like to avoid horrible fps drops which I currently get in majority of my games even when settings are set to low, I also want to be able to use my system for 3D modeling. I want to stay majority with AMD since Im on a low budget. I was going to upgrade my graphics card but then decided that my Phenom will proberly bottle any improved cards since i'm pretty sure it's bottleing my current graphics card gtx 480. (I'm not to sure on if I'm corrected here)

What i'm currently looking at for a new cpu is the fx series 8350/8320/6300/6350 (Which I know my Motherboard will need to be updated to get any of these chips which is something I have very little knowledge about)

For graphics cards I'm not really sure theres alot of options and would like advice on this I generally want to stay quite mid range range around £150-£200 Higher perhaps.

Any information please feel free to ask there is likely to be loads of errors in this since it's late so sorry for that
