Reading OCCT Graphs | Replace gpu or psu

First, sorry form my bad English, i'll try to make myself at least understandable. A couple days ago i started getting "directx errors"(device_hung blah blah blah) with both bf4 and bf3, after 5-10 minutes of gameplay. I've tried every possible fix, including reformatting the computer, with no success. In the process of checking for errors i discovered that the 120mm fan strapped to my video card(XFX 6870, heat-sink accelero s1 plus) had stopped spinning, and replaced it with two more 120mm fans.
But, overheating is not the cause of the problem. It keeps freezing with benchmarks and games, and the temps in the time of the crash are normal(56ºC for BF3 and BF4, 60ºC for kombustor and OCCT and 65ºC for BC2), and the vram usage never reaches 1024mb.
The only games i have here to test after the windows install are:BF3(crash after 5-10 mins, with directx error), BF4(after 5-10 mins, with directx error), BC2(just freezes after 10-15 mins, no errors, though), MSI Kombustor(can run for hours without problems or freeze in the first 20 mins) and OCCT(will always freeze in the first 20 mins, with "error detected). I need help reading the graphs, to now if this a graphic card issue or a psu(cx430v2) issue.
