I have a ATI Radeon 6950 hooked up to a kvm switch. Everytime I switch the away, then back to the radeon, I get a black screen. If I switch the inputs on my monitor, turn it on/off, or unplug/replug the monitor cable, the picture comes back.
I found this article that looks like it might help for NVIDIA...
