What Video/Graphics card should i get?

I run a HP Windows 8.1 64-bit desktop with an AMD A4-3420 APU with Radeon HD Graphics at ~2.8GHz with 6GB ram.
i run two monitors (one 1280 x 1024, one 1920 x 1080) using an AMD Radeon HD 6410D Graphics card. I lag a bit and have to turn my graphics and resolutions way down inorder to play a few games. (Guild wars 2, Borderlands, Day Z-mod &stand alone, to name a few). I play regularly and id really like to upgrade to a graphics card thats $200 or under that would run all of these games at 60fps. is that possible with my budget? What is compatible with my computer? and how do i know what is and isnt going to work with it? Thank you so much!
P.S. Im looking to get some more ram soon, will my current 6gb of memory cause problems with the card suggested?

You PSU will be a major limiting factor in upgrading your gpu.

I am going to guess you have the stock PSU which is most likely a no-name 250w or 300w
So to you could get one of these for your psu,

With those your gaming experience will improve dramatically
the only factor hindering your performance would be your cpu at that point but for the games you mentioned (aside from DayZ) a dual core should be fine.

Pretty sure your cpu will bottleneck the crap out of pretty much any decent gpu, and you may want to add a couple gigs extra ram but for the games you are playing you may want to look into an older amd card. What is your psu like?
