Why does my display have 'block' distortions?

First off I have used 2-3 different video cards so I don't think that is the issue. Secondly I have a cheap PSU, $25, that might be an issue. Thirdly this distortion happens in games when I scroll the mouse fast from left to right.
The distortion looks like sections of blocks on top of each other, imagine your entire screen with lines across it except the lines are the images not matching up. like in the intro to .Hack// where the characters get split from a distortion.
I don't know what's causing it but it's ruining my gaming experience. Thanks for reading!

Lenovo K300-53162HU
Windows 7 Home Premium 64-bit
6GB RAM (2x2GB 400MHz, 2x1GB 400MHz)
ASUS ENGTX-275 896MB (v301.42) using a DVI to RGA adapter
OEM Motherboard
AOC e950swn 1366x768 with RGA cable

 
Sounds like ghosting effect in your monitor, LCD monitors with high latency response will cause that effect which basically happens when your 3d application demands a refresh of the images displayed faster than what the monitor is able to handle.

Sadly the only solution is to buy a better, low refresh response monitor (under 2ms).

I just looked up images of this "ghosting effect" and it sadly isn't the issue. It's more like the image on the screen is being split up horizontally.

I have three monitors which are the one I'm using, a CRT, and an older flat screen monitor from like...2003.
I could try to use the older monitor it's called a Wise Wing F199.

So, REALLY sounds like you don't have the NVidia driver installed, or correctly installed....... (IF this video is to be believed... :) )

NOTE: that dual is going to slow your down from FPS you Could get with the GPU...

Yeah I don't think the GPU itself is an issue but maybe the drivers since I've been having a hell of a time with NVIDIA drivers as of late. I uninstalled all NVIDIA drivers using display driver uninstaller and reinstalled v301.42 and now I just finished downloading v332.21 from NVIDIA so hopefully it helps. If not I'm lost.
As for the Intel Pentium Dual-Core E5800 3.2GHz I think it will affect it in higher in games but I'm only playing Warframe, Halo:CE, Black Mesa, and other free to play games which aren't exactly "intensive".

**Update, I just installed v332.21 and it helped clear up some of the distortion but it's slightly there. It looks smoother when scrolling so I'm happy with that but now when I benchmark and the temp reaches 55 celcius the frame rate drops suddenly from 38fps to 19fps and then when the temp drops the FPS comes back up.

**Update 2, I'm thinking my issues might be related to my dirt cheap PSU. I have had this issue with 3 different video cards and 10+ drivers each card. I am using an off brand called Bluestar 650W Mirror edition.
Also my GPUrequires 2 six-pin power cables and I only have 1 six-pin and a dual-molex to six-pin adapter.
