I want to upgrade my pc, but i dont know where to start!

I bought my pc about a year ago and its time for an upgrade! I really just want it to run elder scrolls online(when it comes out) perfectly @ 60fps and in max resolution on high or ultra graphics, but i don't know where to start! Please if someone knows how to do this at the cheapest possible price i would extremely appreciate it!
Current pc specs!:
Processor: Intel core i7-3770 cpu @3.40GHz
installed memory RAM: 12.0 GB
system type: windows 8 64 bit, x64-based processor
graphics card: NVIDIA GeForce GT 640
chip type: Geforce GT 640
DAC type: INtegrated RAMDAC
device type: Full display device
display: 1920x1080: (32 bit) (60hz)
Also i have a 350 watt power supply (and i know i'll need a new one so please point me in the right direction!)

Also, i would like as much detail on how and where i should get these changes done at the lowest possible cost!

There are two main things you should be focusing on when upgrading this build. Your GPU and your power supply. These should be researched cooperatively. Look for the GPU that you want, look at it's power/pin requirement (i.e it requires a 500 watt power supply with two six-pin connectors), and then get a power supply accordingly. You should also check your motherboard's PCI slots that hold your GPU's to see if it's up to snuff with the latest PCI express technology (x16 lanes and PCI 3.0).

One last thing, that may seem obvious but many people forget to check. Case size. If you have an M-ATX case, chances are you may have to upgrade to a mid-tower to physically fit your GPU in. Just check your case size to be completely sure.
Start by going with a quality psu, like a corsair cx 500 watt, or a seasonic 500watt, but make sure it has 80+ bronze rating, no matter which one you get. these can be had around the $50 range

then as far as a gpu goes, $260 will get you a 760, which will likely not quite max out the game, $300 will get you a 280x which should come pretty close (unless you live in north america where the prices have skyrocketed) and just under $400 is the 770 which will be about equivalent to the 280x

You need 2 items to achieve your goal. A Graphics card GTX 760 or GTX 770. A Power supply is needed as well. Newegg or Amazon are great places to buy hardware. Good Power supply units are Corsair, XFX, Seasonic & Thermaltake. I'd get 650 watt which is about 100 watts more than you need, which is perfect headroom.
