Maximus VI Hero USB/Audio Jack Related Chrash

I am getting a chrash/blue screen of death on my maximus vi hero build. I have a 4770k oc to 4.2 ghz nvidia 760 16 gb of 1600 mhz ddr3 ram and an 850w psu. Sometimes when I plug in a usb device or headphones in to the usb ports ON MY CASE it will cause errors to pop up and the task bar and all desktop icons to disappear. The only way to fix this is to use the power button. 25% of the times that it crashes I get the bsod. Help is appreciated.

I just lowered the clock speed, and it seems to be more stable now. I would like to raise the clock speed higher, what would you suggest? I am running a Cooler Master 212 EVO. I do find it odd that the CPU clock would cause this as I have never seen my CPU get up past 48C on an 80% load for about 5 minutes. Also thanks for you help.

It just happened again, this time I was able to get the crash info from the bosd. Here it is from the technical information:

Ok, so I figured out the problem was my ssd failing. I sent it back to Samsung and received my replacement. This has fortunately stopped the crashing. So here is what I get at 4.2 GHz: 60C under full load with Coolermaster Hyper 212 EVO, and my Vcore is at 1.2V
