Is this a signs of failing Power Supply? Please help me :(

Is this a sign of failing power supply?

But first here is my spec:
Power supply: CiVO 500 watts PSU
CPU: Intel Core 2 Duo e4600 @ 2.4 GHz
RAM: 1.5 GB
GPU: NVIDIA 210
Motherboard: ASrock wolfdale 1333-d667
HDD: Hitachi 80 GB
OS: Windows 7 Ultimate 32-bit

So here are the signs:
1. When its cold, computer does nothing when I press power button, i have to heat it up first at fireplace to get it working.
2. After heating up, turn on, fans spinning, but power LED doesn’t light.
3. Have to turn it off, hold power button for 30 seconds and try to turn it on again, and then it turns on...
4. So after couple of hours working with my computer, it suddenly freezes up, numlock not responding, HDD light blinks abnormally. this is really annoying because i cant save my works.
5. OR IF NOT FREEZING, it suddenly turns off without any warning (no display on monitor), but system unit is still on, (fans still spinning, power LED lighting)

things I tried:
1. try to remove RAM for beeps, it beeps but after a minute. beeping stops, seems like it hangs...
2. remove GPU, problem still occurs.
3. reseat CPU, video card, everthing...
4. remove dusts on my system unit, my system unit is already clean as hell but the problem still occurs.

Should I replace the PSU? or is it something on the unit is failing?
What should I do?

It hangs or freezes at random times, EVEN IN BIOS!!!

ahh ok I see, well another idea to see if it's the reason behind your problems is if you have a known working power supply around that you can use, try that in your system and see if the problems persist.

i think it is already 2 and a half years old, it was sometimes hit by a lightning, but my uncle just fixed it so that i can save my money, he said it was some sort or "diode"? if im not mistaken?

ahh ok I see, well another idea to see if it's the reason behind your problems is if you have a known working power supply around that you can use, try that in your system and see if the problems persist.
