After start up, display randomly disconnects (custom built)

Hey guys, this is my first time asking a question here. Please feel free to ask me to provide any additional details of my issues and I will try my best to do as you ask. All help will be greatly appreciated.

I have a custom built PC that I put together about two and a half years ago. I have not had many issues with it until recently. About two months ago, one of the SATA ports on my motherboard blew out - it would not recognize my second HDD (OS is on a SSD, which was still recognized). Simple fix eventually, switching toanother SATA port on the motherboard.

This new issue seems to be a lot worse. It started just a few days ago. I have not found a sure-fire way to recreate the issue. It seems random, but tends to happen more often soon after I turn on my PC, as opposed to a long time into my PC being on.

A few days ago, one of the fans on my PC sped up and the system crashed. No idea why. The issue now is that sometimes my display monitor will lose connection to my PC. The only way to restore the connection is by resetting my PC. Interestingly enough, a couple of times I have lost USB connection to my mouse and keyboard as well. The display always shuts off first. The mouse seems to follow a little bit afterwards, and the keyboard a little after that. The mouse and keyboard are connected via USB2.0 on the back of the PC. The display is connected via HDMI to my graphics card. I wonder if the motherboard of PSU is at issue here. I sincerely hope it is neither and this can be solved more simply.

Just had the same fan thing occur roughly 5 minutes ago. Started up, logged in, accessed the hardware monitor program you see that I used here (mobo software) and then the fan sped up and the display crashed. I still had USB power to my mouse and keyboard (known due to the lights still being on) and the DVD drive still had power. Restarted and have not had a crash since but have no idea what to expect.
