How should I upgrade?

I have a Dell Inspiron 570 with 4 GB of ram and a 450W power supply. My processor is a AMD Athlon(tm) II X2 250 Processor 3.00 GHz. I was thinking about upgrading my graphics card, which would also require an upgrade on the power supply. Looking to be able to play games like Team Fortress 2 and modern games on around low-medium settings with full resolution. I'd like some assistance on choosing which graphics card/power supply is cheapest, while still holding up to being pretty powerful. Keeping it under 400$ I'd respect.

I would say keep the PSU and upgrade your mobo and CPU to A10 6800k >>>>>http://www.newegg.com/Product/Product.aspx?Item=N82E16819113331

It seems like you are not worried about running the newest games on High or ultra settings, and this APU will do the job.

and maybe grab 8gb of 1600 Mhz RAM with CAS 9 Timings or lower

Later on if you decide you want to run better games you can upgrade PSU and add a more powerful GPU and you got yourself a pretty decent gaming rig.
