Im aware my cpu isn't the best one, but i've getting 22 fps on 1280x720p and even if I change to 1920x1080 there is absolutely no difference.
What could be causing?
I have 6 GB RAM, the graphic card is new.

what game? some games are cpu dependant and your cpu could be the crippling factor regardless of video and ram.

Says so in the title.

Have you installed the latest drivers? It sounds like a driver problem.

So its not the gpu which is restricting the performance, it is your processor.

If there aren't driver, power or any other sort of problem, it might be the CPU. Seeing no FPS gains changing resolutions is a sign of that.

Fixed, I had the monitor cable on the wrong side of the graphic card.
Thanks, I got a rise from 20 to 45 fps avg (max, 1920)

To confirm which causes the issue, you can try running software like MSI Afterburner. Run the game in low settings and try to check the GPU usage, if gpu usage is low then the problem is because of the core 2 duo processor

Im at a loss, the lag came back and the GPU Usage is indeed at 80-100%.
What could it be?
Bottleneck?

Could be a problem with GPU then since it is running at 80-100% even in low settings. Check if its getting over heated. Try to clean the GPU and fans in the computer including CPU fan. I guess you will have enough power supply for the PC
