Do sleek design video cards work?

I am new to upgrading my pc. I know that there are video cards that look like they allow more airflow. I have also seen cards that look like they don't allow airflow at all. Could someone please explain this to me? To me it seems like it would just be stupid to buy one. Could someone please explain? Thanks

That is the reference design.
The fan sucks air in, flows it through the heatsink and out through the back mesh.
Aftermarket coolers consist of larger heatsinks and/or more fans.
Some designs also allow copper heatpipes(more conductive) direct contact with the GPU, allowing for better heat dissipation.
That also allows for overclocking, and a overall better cooling.
But it comes at increased price.

There's nothing wrong with cards like that but one's with larger coolers allow you to overclock you GPU easier. That card follows closely to the reference design that Nvidia puts out.

The one you linked has a fan that pulls the air from inside the case and pushes it through the card out the vent in the back near the video ports. I am sure it flows a fair amount of air, but might be noisy, maybe.

I've had a PnY card in the past and liked it.
I also wouldn't call it a sleek design, as it looks like a double slot like many other designs.
