in mouse properties do you have "enhance pointer precision" checked? if it is then mouse speed on screen varies with the speed you move the mouse on pad (it will cover different distances)
or, in the game you are playing do you have sensitivity different in the x or y directions?
is your frame rate in the game high enough (not stuttering or anything right?)
