According to CrystalDiskInfo, one of my drives is under "Caution", specifically because of Reallocated Sectors Count. Should I be worried? Is failure imminent?

You should be alright for now. Make sure your important data is backed up and in a few weeks run the test again. If the number of reallocated sectors has gone up then you might have a problem. This is usually caused by defects in the disk where the disk will mark them as bad so they cannot be used. The larger the drive the more of these tend to show up, if it is a small drive and there are allot of them then the drive may be defective. Is this a drive you purchased and installed or is it a drive that came with your system?

Check the reallocated sectors and pending sectors daily. If the numbers are increasing you will want to backup your data and replace the drive. Sometimes drives with bad sectors can last days, other times they can last years, and sometimes they die quickly. It comes down to how comfortable you feel with the situation. In any case, a backup is always a good idea.
