Can anyone help with my problem while gaming?

Whenever my framerate drops below my monitors refresh rate, it feels nearly unplayable. Another example, let's say I'm playing at 75 fps and my framerate barely drops to 71, the difference in smoothness is ridiculous. Even though it only dropped maybe 3 or 4 frames, it feels like it dropped 30 or more. This happens in Bad Company 2, Battlefield 3, Black Ops.

I've re-installed video drivers.
My CPU temps run between 52C-56C while gaming. My GTX 560 runs at 55C while gaming, so it's not overheating
I've defragged my hard-drive
I've ran CCleaner
I've changed NVidia Control Panel settings
I've used V-Sync, and Adaptive V-Sync
Used Razer Gambooster
I even upgraded to Windows 8.1

I've been looking all over every forum section I could find and it seems no one has found a solution to this problem. Could it possible be my Hard drive?

I took a screenshot while running HWiNFO64. Does everything look okay? I notice that my CPU core frequency is lower than what it should be, but think that's because I have 'Cool and Quiet' enabled in my BIOS.

My monitor defaults to 75HZ and it's set to that globally in the Nvidia Control Panel.

Here's my specs
Motherboard : ASRock m3A770DE
CPU : AMD Phenom II X4 955e @ 3.2 Ghz
RAM : 8GBs of DDR3 RAM
Video Card : NVIDIA Geforce GTX 560
PSU : Corsair GS800w
Monitor : Dell 19' 1440x900

Could it possibly be my hard drive failing. I'm not sure if this is normal, but when I launch Battlefield 3 and go into the task manager it shows my Disk Usage is at 100%.

not likely because its using all of your disc speed to load up the level.
