My mobo is an older asus P6T Deluxe with no support for 3.0 USB, but my case is a newer corsair with front mounted USB 3.0 ports.
Is there a card out there that supports the modular plug for the front ports and give me a couple of 3.0 ports in the rear?

I would recommend getting a USB 3.0 to USB 2.0 adapter to be able to plug it into your motherboard. I do not know of a USB PCI card that you can plug a cord into.

Yes, look for a PCI USB 3.0 with an internal header.

They can be found on EBay, Amazon, Newegg, and elsewhere starting around $20 for two external ports and one internal header for the 19 pin front panel blue wire.
