It has done this for a couple of months and I honestly don't know what to do. I need some help, can anyone tell me what can be causing this and what can help fix this?

Yet I have tried a Memtest86 which came up empty.

It happens randomly after 1-3 hours of time on the computer and then suddenly it freezes.

DxDiag showed this

I know it's in danish, but has all the part names and such.

That is a 600 w Raidmax psu. I can't tell the exact model of your graphics card. It could be either a HD 7950 or 7970. My initial guess was that you have an under-powered system but your psu should be enough to provide either of these two cards with enough wattage and amperage for stable operation. Your graphics card should have 2 power connectors connected to them, make sure of that. Also try to update your GFX card drivers.
Another possibility is that your GFX card GPU is overheating. Download a software like HWmonitor and check your gfx card temperature.

Do you have integrated graphics on your motherboard?
I checked your motherboard and it doesn't have integrated graphics.
I have noticed something. Your CPU core minimum voltage is kind of low same with your GPU voltage. I am not sure if it is normal for them to be at these values. There is a possibility that you power supply is failing. There is a known problem with FX bulldozer processors like yours as well when they run under windows 7 and you need to install a hot fix update for it. I think this is kind of a remote possibility, since until recently your didn't have problems with your PC, but you won't lose anything by installing the update:
