Today I was installing software from my computer to my phone and suddenly my screen went black. I saw the AMD boot logo and then instead of booting to windows, I got a message saying it couldn't find an OS. So I threw in my windows disk and tried a boot repair with no success.
I popped the drive out and put it in an enclosure and put it in my work computer and was able to see all of my files.
So I put it back in the PC, booted in to bios, (played around in there for a minute but didn't see anything helpful so I exited without saving. After exiting the BIOS Windows started right up.

Fast forward an hour I was copying a file from my PC to an sd card when it said "Can't copy file due to an unknown error" a couple of minutes later, explorer crashed. I tried a hard power button reset and after the AMD boot screen I got that same message informing me that the OS was missing.
So I played around in the bios for a minute. this time the bios didn't even recognize my HDD.
I popped the cover and pressed the HDD to be sure it was tight. No boot.
Finally I took out the HDD and let it sit for a minute.
When I put it back in Windows booted right up.

Why is my HDD disconnecting istelf.
How do I tell if the problem lies with the disk or with my PC?
If it's the PC, where is the problem? is it a bad connector or is it a software issue?

Start by testing the hard drive with the manufacturer's diagnostic software. If you put the drive back in to an enclosure and plug it in to a working PC, you'll be able to test it from within Windows, otherwise you'll need to test it from a bootable CD that you have to create.

Just occurred to me that you may want to first try changing the internal data cable for that drive, Existing cable could be faulty, not the drive.
