OK, definitions:
Modem and router.

The modem talks to the ISP and provides one and only one IP address. That address can be passed to a router.
The router can then serve up multiple internal IP addresses. Generally in the range of 192.168.1.xxx.
Often, the modem and router functions are in the same physical box.

How many RJ-45 ports are on the back of this 'modem'? What make and model?

aRE YOU SAYING THAT IF YOU CONNECT YOUR PC TO THE MODEM, OTHER DEVICES CANNOT ACCESS IT through WiFi? If so, then no, other devices should still be able to access it. With wired devices, it depends on the number of RJ-45(Ethernet) connectors your modem has.
