Graphics card shuts down randomly.

Hi, over the last few weeks when ever i have turned on my computer, around 10 mins later the screen randomly goes black and the after a few seconds my computer fans keep going but my skype stops. i used to be able to i turn off my computer and after around 5 mins it would work again. but lately its been getting a lot worse and my screen dies after only a couple of minutes and wont work again as long as i am using the connecting the graphics card rather than the motherboard. then the next day when i get back from school it works again for 2 mins and the same thing happens until the next day. sorry for my ignorance and thanks in advance.
i should also add that as soon as the screen goes black the computer sounds almost twice as loud and the red hdd led on the top of the case lights up constantly.

Fans working hard is a sign of overheating.. Please post your GPU/CPU temps when idle..

At this point in the answers I'm going to take a guess that the power supply can't handle the system needs over something in your rig is seriously broken. List your part list and if the build is self constructed or built for you. Also, to help eliminate the possibility of a couple answer unplug all of your ssd's and hdd's and attempt to keep bios running for the an extended period of time.
