Games:
Civiliation V
Garrys mod
Minecraft
DC universe and other steam games, when I watch youtube it is fine until I maximise it, then it pixilates. I have cleared and scanned many times for viruses and removed them.

1) Whats your hard ware. Most importantly is it robust enough for the games your playing.
2) Are the drivers up to date for your graphics adapter.
3) are the games compatible with vista

After these simple steps are addressed with the problem persisting you will need to test other parts.
Try switching with old parts like a psu, ram etc, you or a friend may have lying around

good luck hopefully the fix is cheap

Sounds like you are playing games in a non-native resolution for your monitor. If you expand videos to full screen but they are lower resolution than your monitor, they will look a bit off. Same thing for games. If you watch HD YouTube videos and go full screen, I am betting they look a lot better than others.

The other issue is that you have to remove viruses many times, that is BAD. You're doing something unhealthy with the system, either visiting sites that are easily hacked or downloading torrents.
