First I was playing a game, now I've done this a lot since i got the GPU and in-fact had just gotten done running a Heaven Benchmark to help someone with their results. Anyway never had any problems with hitting 99% usage. I went to play Planetside 2 and wasn't getting above 50% usage... that was even high for the average kept dropping to around 40%-45%. I figured maybe just a hiccup and decided to restart after restarting I got my middle monitor saying "out of range" while the other two queued up just fine.
