Graphic signal loss

Hi, I have an Hd Radeon 7870, with my drivers up to date, and I've been having signal issues since I purchased my PC a year and a half ago.

My problem is the following, at random times my screen freezes for a second, the music stops and the signal is lost, then the screen turns black and then it recovers and a message says that amd software stopped working but it was recover appears on the screen.
Also, after some time playing a game (it doesn't matter wich one) the screen freezes the sound is lost and inmidiately the screen goes black, the PC still works, I can hear it working and if I turn it off and restart everything works fine, but this happens randomly some times is after five minutes of playing some times after an hour... Same as with the sudden losses of video, and is pretty weird because i have an HDMI switcher connected with a console and a TV reciever and this problem doesn't happen with them.

Things I've tried:
-Connecting it without the HDMI switcher
-Two different HDMI switchers, one with power supply
-Different cables
-Different drivers (I have updated my drivers a couple of times since I purchased the PC)
-Different HDD ( I have an SSD now)

I honestly don't have any idea of what I can do now, I've seen a couple of threads with problems like mine, but there is never a definitely solution, so I hop you people can help me.

This happens with a bad Overclock to. If you have OCed the card remove it and see if the issue persists.

You can test the GPU with OCCT set on the GPU test with error checking enabled. If it errors there is an issue with the card whether it is a Bad OC, Bad Driver or bad card. Since the issue has been around through other drivers it is either a bad OC or bad card.

Ok I've tested it for two hours with error checking enabled and my monitor crashed like it does when I play a game, so, should I asume that this is a GPU issue and should I replace it? Because I had to shut down the PC using the button because it was unresposive.
The signal loss has happened on my TV reciever too a couple of times but I didn't mention it because it happens very few times, no issues with the Ps3 though.
Also, is there any way of using OCCT to have an issues log? If it's so, I could update it here so we have a better overview of the problem.

before the crash did you get any errors from the card? also when you tried the other drivers did you do a clean install. This means wipe out all other AMD GPU drivers and do a fresh install. I use Driver Fusion to do this because it works on other drivers to and not just display drivers.
