Black Screen Crash - No Signal Input/Sound Loop

Hello again, a while ago i posted with this same issue. My PC was overheating (around 90 degrees C.) Eventually the Intel heatsink pins broke, instead of ordering new ones of those I gave up and went for the hyper master 212 EVO. Temperatures are now fixed (around 70c maximum.) However, the black screen crashes remain.

Specifications:

O/S: Windows 7 64 bit
CPU: Intel Core I7 2600 (Non over clocked, non K.)
RAM: 2x Patriot 4GB DDR3
GPU: AMD Sapphire HD 7870 (Factory OC)
PSU: 750W CIT
M/B: H61M-S2PV
HDD: WDC WD10 1TB (Storage Only)
SSD: Samsung 840 EVO 250GB (O/S and Software + a few games)

Here's a list of things I've already tried/checked:

Temperatures: Not the issue, everything is running cool now.

Drivers: Full un-install of graphic drivers in safe mode with driver sweeper, no help.

Small Underclock of Graphic Card (100 on both clock and memory) - Seems to possibly be slowing down how long the system takes to crash, but it doesn't solve the problem.

Voltages, here's the reading from BIOS:

CPU V Core: 1.118
DRAM Volt: 1.524
5v: 5.124
12v: 11.926
CPU VTT: 1068

I've noticed the system mostly crashes under extreme load. Such as 2 or 3 games running at once. Could anyone give suggestions of what could be causing this problem? My main suspicions were PSU or Memory, however definitely not certainly..

That PSU does have 2 12V power lines which isn't the best for stability under high loads.
I would recommend this:
http://www.corsair.com/en-us/tx750w
It has a single 12V rail for stability and performance plus its also very efficient under high loads.
(If still in warranty I would return the item as faulty)
