Computer won't boot normally, vertical red columns, random USB ports not working, and more

Current build: (Win7 64-bit)

AMD FX-8350 @ 4.4GHz
Sapphire Radeon HD 7950 3GB
ASUS M5A97 AM3+
CORSAIR Vengeance 8GB DDR3 1600
Western Digital WD Blue 500GB 7200 RPM
Creative Audigy FX Sound Card

and a Corsair 850W PSU which I bought 5+ years ago that I can no longer find on newegg/amazon.

Issues:
To start it all off, my display driver(Catalyst latest stable driver released back in December) crashed a couple times within 30 seconds followed up by a completely white screen with black lines running vertically through it. On reboot I noticed I now had two red columns running up and down my screen on bios which then turned into a black and red checkerboard in safe mode.

Now, every once in 5-7 boots I will no longer have these columns and everything will function normally up until 5 minutes in windows. Once I hit this mark my screen will turn a random color with lines running through it once more. One thing I did notice when this happened was that my audio still worked fine. Other issues I have is that either my mouse or keyboard will not work past the bios menu and it is never the same item on the next boot.

Attempted fixes
I have reseated my gpu, cleaned everything out, checked connections, attempted system restore, and uninstalled AMD drivers With that being said I am hoping for some insight as to which component is the most probably culprit here. If it happens to be my GPU then I have 12 days left on my warranty to get it fixed up. I also have a new Raidmax 530W PSU in the event it is my PSU finally dying.

Update:

After leaving it sit overnight I was able to complete a few rounds of BF4 with no incident. A few minutes after closing the game and just browsing, my screen started flashing a random color with lines running through it a couple times quickly followed up with my display driver crashing. I checked for artifacts as well and in a 20 minute scan found none. I can now boot normally just fine but crash anywhere between 5 minutes and hours later.
