PC Crashes when gaming

Hey guys...ive been having this problem for awhile now... everytime i game the monitors black out and i have to force shutdown to turn it off
my specs are:
Intel Core 2 Duo @ 2.8ghz
6GB Ram
Asus HD 7790
Samsung 1TB HDD
WD 320GB HDD
WD 500GB HDD
Raidmax 500W PSU
I bought the psu about a week ago

Grab GPU-Z from here http://www.techpowerup.com/gpuz/

Run that program while you play a game. Enable log to file and continue to update while in the background. run a game till it crashes. post the last string of numbers before it crashed.
