Bad performance, looking to get some minor upgrades... (Ideas on better components?)

Hello everyone! I've been looking around some sites for info to increase my computer's effectiveness in games. I've been using it for quite a while, and shes very good at running most things that are basic games/programs. While it has a good amount of ram and proccessing from what I can see, it doesnt run new games on anything except 640x400 and on low. Skyrim is the ultimate lagfest at the lowest settings and a few games that I've started opting-in for on beta testing don't even let my computer start them now-a-days.

I'm thinking of getting a better graphics card, but I'm not sure if thats what I need. I'm no good when it comes to building computers, any help would be great! I'll put everything down here that you might need to know -

- Rating of 4.1
- AMD Phenom II x4 925 processer 2.80 GHz
- 8GB of RAM (7.24 Usable)
- 64-Bit system
- Model: HPE-112y
- ATI Radeon HD 4350
- Realtek HD audio

Ask me for any other bits of info if its still not enough! I'd like to get parts that optimize it specifically for gaming and video!

I would change the processor to: AMD Athlon x4 750K, it is faster in single-threaded applications. But you can leave the Phenom if you want.

The best GPU for the processor IMO would be either: AMD Radeon R7 260x or NVIDIA GeForce GTX 750Ti.
