Extremely high temps on H60 and i-5 4670K

On a new miniITX build using a i5 4670K and a GA-Z87N-Wifi board with a H60 watercooler I get a temp of 91c when booting (in the UEFI bios) with no overclock. I'm using a Corsair 250D case.

It idles at 40c-60c. when trying Prime95 it goes to 99c and when AIDA64 its similar (96c-98c).

I've not done that much testing as I know these temps are very harmful to the CPU over time.

I am not sure whether the cooler is a dud. As per Corsair instructions I have plugged the pump to the SYS_FAN header on the mainboard and the fan to the CPU_FAN header. I've also taken off the watercooler, cleaned the compound off that and the CPU with Akasa cleaner and applied new TIM to no avail.

I'm not sure if I have the radiator & front fan set right either: the radiator is inside the case, then the fan which is acting as an intake (should it be the other way around)?

Could someone advise if there is anything else I could try?
