Lower voltage will never cause damage, period. The worselt it could do is not give the card enough power and have it shut off.
Higher voltage might, but you're well within safe boundaries for that.
Different programs are going to give you slightly different reading, yes. Seriously, don't worry too much about it - small fluctuations like that aren't a huge deal, and are mostly just caused by the sensors.

Lower voltages CAN cause damage if it goes into a buck or boost converter (which a GPU VRM is) because it causes the current to go up to compensate. That said, you are still in spec so there won't be any damage.
It is also not unusual for the GPU voltage to be a little lower than the motherboard voltage because the high current usage and wire voltage drop

Looks like carbonization or oxidation. It could be caused by less than perfect contact but it isn't really damage.
It will probably wipe away with a pencil eraser but I don't expect it to cause a problem anyway
