CPU: core i5 4200m
ram: samsung 4 gb 1600 mhz
GPU: amd R5 M230 and intel 4600
hard disk : plextor m5s 128 gb ssd
I try many games on medium setting and the performance was very bad

You're still looking at a lot more than just opening the case and swapping cards like you would in a desktop. The chip is more than likely going to be integral to the system. The time, effort and trouble you'll go through - not to mention risk to the board itself and voiding the heck out of your warranty - would probably be better spent getting a laptop better suited to gaming.

However, if you're the type that feels comfortable digging in that far, then by all means. But there's a reason why you won't find a lot of hands-on instructions out there to help you - most people are not able to do it. That being said, I have no idea if those two chips are the exact same connection type, compatible with the same firmware and/or if they are...

If you get through all of that, I'm sure you'll be able to tackle the heat problem. I've been modding things for years, and I don't have the guts :D If you get it to work, make sure to document it for others.
