My graphic card is not being recognized!

Hello,
I've been using an integrated video card for a while, then i figured it's time to buy a new GPU. So I've bought a Gainward GeForce GT 610 2GB. I installed it but it didn't work at the beginning... Then I've found something telling me that i have to disable the onboard card first. I went to BIOS and I set it to PCI Ex. The new GPU worked fine for about 3 days, then started showing the No Signal error again ( shows "No Signal" message for 4-5 times then the screen goes black and the led turns orange), so i tried switching to PCi Ex again from BIOS but this time won't work. I don't know what the problem may be since the card was working fine yesterday. The integrated card runs fine. Any suggestions?

As I said - this story has gone on too long to remember what has been said.

So if your card is working in another pc, it's something else in your pc. Try a new psu. Try a new mobo.
Yep - but UEFI only comes with new mobos. All your stuff is old except the gt610. Your PSU is crappy. Your ram - ddr2 and only 2GB of it - is very poor. Any of these things could cause a problem.
Your PSU might have failed with the extra load from the gt610. It sounds like your motherboard switched the Bios back to integrated graphics all by itself. Is that so? I can only guess at how your Bios is laid out. Is there something where you not only have to turn on the pcie graphics but also turn off the integrated graphics?

I can't switch the integrated card off, i only can set it to Auto/Enable. The Bios is set to boot from PCI Ex first, but when i connect the GT 610 the monitor shows me the No signal error for about 4-5 times then the led turns orange, tha fans are spinning too.
I think that the PSU may be faulty as well, with those 2GB RAM i ran a 9500GT with no problems, but this PSU i installed it recently and it may be the problem, I'll try to find a better PSU to test the card.

My GT 610 wasn't connected in the PCI slot at that moment, that's why, I was connected to the integrated graphics.
And here's a pic with the voltages shown in BIOS with the same PSU:
