Black screen while playing games

Hello! I've got strange problem with my PC and can't figure out what's wrong.
Today I was playing Minecraft with my friend when suddenly my screen got black. Strange thing is that I could still hear music from game and my friend on skype, we even kept talking. Since my display said "No signal detected" I tried restarting PC but on startup I heard 4 long beeps and one short. Dr. Debug gave me code d6 which is related with GPU not being recognized. I pressed power button to shut it down and then everything was normal. Tried playing again, 5 minutes later exactly same thing. And now I can't play Minecraft for longer than 5-10 minutes. We've been playing whole day yesterday and didn't have any problems.
My rig is about 2 years old. It's strange because I haven't been playing minecraft a lot before. I've been only playing League of Legends and never had that problem. But I also played some Hearthstone and got black screen 2 times in one week. I tried updating GPU drivers (I even installed beta drivers), but still same. I've ran furmark for 10 minutes, no problems. Tried running memtest, no problems. I tried to monitor the temperatures and they were fine at the moment of crash. Also, I didn't get any dump files.
3 weeks ago I did fresh install of windows, so all drivers should be up to date.

I already tried setting the fan speed manually in Catalyst Control Center, but didn't help. The maximum temperature I get in Minecraft is around 60°C. In Furmark it reached 75°C but didn't have any problems.

Small update. I still have problem with black screen, but in Event Viewer I've discovered that at the moment when black screen occurs this warning pops out. "Display driver amdkmdap stopped responding and has successfully recovered." But I still have to restart the PC for it to work.
