I just purchased a Radeon 6350 DMS59 card.
http://www.amazon.com/VisionTek-Products-Retail-Graphic...

I have 3 monitors, but only 2 are working. I have one monitor plugged into the Display Port using the mini to fullsize adapter that was included in the box and a DP to DVI adapter. This monitor is working.

The other 2 monitors are plugged in using the DVI splitter included in the box. Only one of those monitors is working.

All three monitors are showing up in Windows. When I go to the catalyst drivers, I see all monitors as well. The DP one, is showing as Monitor 1, the #1 monitor on the DVI splitter is Monitor 2 and the #2 monitor on the DVI splitter does not have a number. If I click the triangle icon and click expand on that monitor, it says I need to disable either Monitor #1 or Monitor #2. How do I get it to let me use the 3rd monitor?

The product page only mentions "Single slot, Dual Monitor support" Is it possible that it does not support 3 monitors?
http://www.visiontek.com/dms59-sff/hd-6350-sff-1gb.html

Anyways, you could also try using an HDMI to DVI cable for the 3rd monitor and not use the DVI splitter. That gives you the following setup:
1x mini Displayport to Displayport
1x DVI
1x HDMI to DVI

If that doesn't work maybe it doesn't support 3 monitors; return it and get a different model/brand.

Also, might I ask what model computer (or motherboard) you have? It is possible that the 6350 is slower than your current integrated graphics.

This is the model of the card... it says it supports 3 monitors.

The computer is a Dell Optiplex 7010.

EDIT: After reading this it sounds like I need a Mini-DisplayPort to DisplayPort cable to plug that into the 3rd monitor. Does that sound about right?

Oh, sorry about that. Thanks for the info.

So, here's what it says:
Quote:
** Eyefinity Connectivity Requirements: ATI Eyefinity technology is supported on this model when in all three output connectors are in use. The following restrictions apply:
• A maximum of 2 legacy monitors (VGA, DVI or HDMI) can be enabled simultaneously, provided that each monitor is connected either directly or via a “passive” DisplayPort™ adapter/dongle. “Passive” adapters/dongles will NOT support more than 2 legacy monitors.
• To enable support for more than 2 monitors, “active” DisplayPort™ adapters/dongles are required (or monitors with direct DisplayPort™ connectivity must be used).

So, my guess is you might want to try the following setup:
1x mini Displayport to Displayport port
1x HDMI to DVI or HDMI to HDMI
1x DVI to DVI

Only one monitor needs to be Displayport. You said you are already using Displayport on one monitor though, right?
What are the plug options on each of your monitors? ... or what are the model numbers for each monitor?

(again sorry)
I just noticed....
Are you chaining two cables together like this:
mini Displayport to Displayport; then: Displayport to DVI?

If so, this is why the setup does not work. You have to use a 100% Displayport chain. The plug on both the video card and monitor must be Displayport and the cable must also be only Displayport.
If you convert Displayport to DVI or HDMI, then you have to buy an expensive active converter so the 3 monitor setup will work.

Why?
(may not be entirely accurate on some words)
The AMD card probably only contains two TMDS or clock signals. This is enough to drive 2 monitors over HDMI or DVI.
Unlike HDMI or DVI, Displayport does not need this, so you can use Displayport for the 3rd monitor.
However, if you convert Displayport to DVI or HDMI, the signal works like a normal DVI or HDMI instead... which needs a third TMDS or clock signal, which you don't have. Thus, you have to buy an active Displayport to DVI or HDMI which provides it's own conversion chip in the adapter and adds a 3rd TMDS or clock signal for the monitor but lets the graphics card treat is as a Displayport connection.
