Nvidia drivers aren't working with my nvidia graphics card.

I have a GeForce GT 640 Card and I was having trouble with some of my usb ports not recognizing devices so I went to uninstall some of the drivers. Mistakenly I uninstalled the video driver as well. After reinstalling the latest drivers for nvidia i noticed that after the restart the display wasn't filling out my 1080p monitor. So I just did a clean install of windows 7 and installed all the latest drivers and it was working until I did a restart and then the drivers uninstalled themselves again. I also went in to Device manager and noticed that the code 43 was in effect. "Windows has stopped this device because it has reported problems"

Here are my specs:
Motherboard - ASRock 960GM-VGS3 FX AM3+
Processor - AMD Phenom II 3.2 GHZ Quad-Core
Hard Drive - 250GB 7200 RPM SATA Hard Drive

Any help would be much appreciated!

I tried doing both just now, reset bios settings and removed the battery. No such luck.

Could it be that the video card just died even though it was working not a day or so ago?
