When your PC is off, does the fan turn easily by hand, or do you feel resistance? If the latter, its bearings have probably worn out and locked up.
The HD5570 is not a strong card by today's standards. You'd be able to run a HD7750 or GTX750 without upgrading your PSU, and see considerable improvement in frame rates and in quality settings.

im not at all familiar with the card, but is the fan super dusty? could just be clogged up... remove the shroud and clean it.
if you had to replace the fan, sometimes you can and sometimes you cant. depends on the card/manufacturer

as noted above, might just be easier to drop in a 750 since you can get one for around $110... and it'd be a big boost

While you can buy a replacement fan, finding one with proper fit might take some research then some mods.
If you can, your best idea is to replace the card. Even the R7 250 or HD7750 is a big step up in performance (make sure you get one with GDDR5, not DDR3).
