Download speed sporadic/2 mac's same device

I am having some issues with my home network. Recently I bought an Asus RT-AC66R. It worked great for awhile. Recently I have seen the download speed slow down at random times. We kinda live in the sticks and have bad speed to begin with, but I can run 5 tests within 5 minutes and get anything from 3 Mbps to 400 kbps. After a router reboot today, I got 2.5+ Mbps over a 30 min period running a speed test at least 5 times. Then I turned my laptop on and it slowed down to about 1.5 while running a virus scan. I ran a speed test at the same time on my iPhone and laptop and got 2.5 on the laptop and less than 1 on my phone. I have a range extender set up, and if I'm connected to the router and it slows down, I can connect to the extender and it speeds back up again for a little (i.e. Enough time to finish a video with no interruptions), Also, not sure if it's related. But my iPhone is listed twice on my router. 1 MAC address is correct, the other is not. I would really like to figure this issue out. I will have our ISP tech come out soon to check things out. My phone has acted weird recently using large amounts of data at random times throughout the day when I'm not doing anything like at 8:15 am & pm for a week straight everyday same exact time. Please help, thanks.
 
What modem are you using? The router is good

Any other people using the router? Who by cable and who by wifi?

What wifi card/chip is in your laptop? post link

If there's a few people sharing the 3mbps then it'll jump down to 400kbps easy.

You might be getting devices showing up on the network from the router and the extender

The modem is as new as the router. It's a netgear -ADSL2+ Broadband Modem
Model: DM111PSP

3 other people in the house with laptops/phones. All wifi no Ethernet. No one plays Xbox/ playstation etc...

I think the NIC in my laptop is an Intel Wireless N 2230 http://www.intel.com/content/www/us/en/wireless-products/centrino-wireless-n-2230.html

I understand 2 occurrences of the same device via router and extender but not having 2 different MAC addresses.

We are thinking of switching to Cable, checking on prices. I've never liked AT&T DSL
This just recently started happening, everything was fine, no big changes then all of the sudden, it started
 
Well there's your problem. Similar to me - too many people on the wifi. At least you get a decent service. I only get 1.3mpbs and no.3 son is right into gaming while youtubing. My only option is to get better internet. I'd say you're in the same boat.

That seems like the logical answer, but I'm not sure thats it. I was doing it earlier today when I was the only one home. Before I reboot the router, it was down under 1 mbps. after the reboot, it was 2.5+ consistantly. It just went back down for a few minutes a little bit ago

I've got the modem connected to the router, 2 individual pieces. Unfortunately, its an older house so that's about the only place for it. I've it drop while stand in the room right next to it
