I5-4670k exceeding max clock rate

Hello! So I recently got an i5-4670k to go with my MSI Z87-G43 mobo. I began overclocking it and noticed it crashed instantly when I saved it to 4.3GHz and it couldn't take a benchmark test without blue screening at 4.2GHz. So I was content with a 4.1 OC. I played some games and one day it just froze and crashed. I was worried (obviously) and went to check out what happened. Sure enough, my CPU was exceeding the set 4.1GHz max ocassionally by 100 GHz, and I'm assuming it just hit 4.3 and crashed. Any help or suggestions on why it is exceeding the max? PSU is 750w and shouldn't be a problem. Max temp was 73C avg of 65 at 4.2GHz under full load.

Please list your complete oc settings that you've manually adjusted. How did you acheive your oc? If you used a software suite it may have something like the load line calibration set to high, 7, or whatever they call it and this may help with causing your system to "over shoot" a little and can be pulled back. However, I'm not 100% certain that's it. Please clarify the above mentioned details to the best you can.

I was using intel's extreme tuning software and I OCed it by adjusting the voltage to adaptive and then set the cores to the GHz I wanted and put the cache to 100 GHz less than the OC
