Solve my mystery

I was playing a game of LoL the other day, after the game started I immediately noticed horrendous stretching on the screen. Soon after the start the game crashed and every time I got back in it would continue to crash. Eventually I blue screened and when I booted my PC back up I was greeted with this. http://i.imgur.com/MkqNVOW.jpg

Since then i've done a clean install of windows, I've removed and installed graphics drivers and nothing has changed. In my device manager in GPU properties this is what I see. http://i.imgur.com/zU78F70.jpg

I need some help figuring out what is wrong and with what part of my PC and can it be repaired.
PC specs:
Motherboard: ASUS P67 Sabertooth GPU: Nvidia GTX 580 CPU: Intel Core i7-2600 @ 3.40GHz

when's the last time you cleaned the heatsink and fans on the cpu and gpu? or pulled the heatsinks on either and cleaned up and reapplied new thermal paste?

is the power supply failing? what is the PS?

is the monitor cable tight at both ends?

did you try a new cable?

first glance would say card is dead or dying...... could be one of the memory modules on it.

older machine ( parts ) carried over, etc........................... first things that go are ram/ps/and graphics. if you can find someone to borrow a PS or card from, or take your card to someone with a decent PS............. you'll do yourself some good before going out and dropping money on something............... your motherboard, does it have another pci-e slot? if so, try it.
