Weird rectangles on some games

I have an Nvidia gtx 660

I first noticed this problem back when i first bought my computer and I installed tera. When I was playing the game these weird rectangles would come up. I just thought this was a bug with this particular game and proceeded to stop playing the game... I then played chivalry... the same rectangles came again. They also appear on outlast and saints row 3.

They are not furmarks from the graphics card overheating since I have tested for this. I have also uninstalled/reinstalled current and previous nvidia drivers to see if that did anything but nothing worked.

Does anyone have any idea what they are and how to get rid of them?

Graphics card defective likely, specifically the video memory.

If you DOWNCLOCK the cards memory (GPU frequency likely won't matter) you'd likely see this disappear.

Interesting you mention FURMARK because I tell people not to use this program as I still think it's possible to damage some cards despite thermal management.

Do you have another 16x PCIe lane to try? Or even another computer to try the graphics card in?
Have you researched whether there is a BIOS update for your motherboard or updated chipset drivers?
You may also want to research whether there is a BIOS upgrade for the GPU, though I'd upgrade that as a very last resort.
Is your PSU under stress, able to provide enough power, or overheating. Similarly is your CPU overheating?
