Whats Wrong With My PC?

Hi, I've had this HUGE problem with my PC for like 4 months now and I really don't know what to do. When ever I start it up with the display connection in the graphics card, on any input (VGA, DVI, and HDMI) the screen either just stays off or it turns on and displays it untill it eventually freezes the whole PC and I have to restart it. Right now i've got the VGA display in my motherboards built in graphics and it displays perfectly except I can't play any games :( 
PC Specs:
Motherboard Asus M5A78L-M LX Plus
Graphics Card (I THINK) nvidia geforce gt 610
8 gig ram
980watt 230volt power supply
500gb Harddrive
normal cd/dvd drive
CPU QuadCore AMD FX-4100
------------------------------------------------------------
Please Help, all them parts are a year old except for the power supply, the PSU is like a month old

Hello...1) Maybe your cooling fan on your gt 610 is not turning? slow rotation? or Dirty?
2) try removing your 610 and cleaning the connector with rubbing alcohol and re-seating it.

The gt 610 is about the bottom of the list of performance as a gaming card.
