Will it be possible & worth upgrading?

Alright, so I have an old workstation build I guess, and I was thinking off duing a little upgrade so I could play Battlefield 4 on it. I dont know the full specs but I know some:

CPU: AMD Anthlon x2 215 Dual Core
RAM: 6GB
GPU: Nvidia GT 320

Thats pretty much all I know, I was wondering if it would be possible for me to upgrade to a GTX 750Ti and play some games on it, or do I need to buy a fully new system? oh yeah I think the powersupply is 250W, and I was planning on getting the EVGA version, so I dont need any power connectors. Thank you for any advice!

If your BF4 play is multiplayer, then the cpu will be inadequate.
For single player, a graphics update will do the job.
If the 250w psu is of decent quality, then it should be fine.
since it is working now, I would try it. If you have a problem, a 400w psu like corsair is often on sale.
