I need advice about upgrading a VERY old rig

So my grandma decided she didn't want her old desktop because she got scared that Windows stopped supporting XP and Vista (I understand the flaws with this thinking. Just ignore it) so she gave it to me when I said I would like to play around with it a little.

What I want to do with it is replace the CPU, the RAM sticks, and the Graphics Card but keep everything else, including the case, motherboard, and PSU. I want to replace them all with MUCH better parts like move from a:
-2.5 GHz single-core CPU to something like a 2.5 GHz quad-core
-two 512 MB RAM sticks to four 2 GB stick (I've checked, there are enough ports)
-Move from an old graphics card to a new gaming one (I don't know if you could tell but I know very little about graphics cards)

I haven't done anything with it yet and I thought I would ask to see if anyone had any advice or saw any issues with the plan. This is the first time I am trying to do anything like it so bear with me if I've overlooked some major flaw

Anyway, any advice would be greatly appreciated so that I don't fry one of my favorite play things.
