Eyefinity not working

My eyefinity stopped working, I Have 2 SAPPHIRE HD 7870 GHz Edition OC 2GB in crossfire and have 3 iiyama monitors set up, when i first set up eyefinity it worked fine playing bf4 and got 60fps on medium settings and when i got out of game the left screen went black all of a sudden so i swapped around cables and there was always one monitor black so i updated the amd software and now it only gives the option for setting up eyefintiy with 2 monitors even though there are 3 connected, i have swapped around cables at the back using 2 dvi and a mini display port all coming from the same GPU and still no results. Any suggestions on a fix?

That link is broken, but I', just going to assume it's an active adapter. I have no clue why your third screen stopped working.

Try running Display Driver Uninstaller in safe mode, then reinstall CCC and set up eyefinity again.

Is it an active adapter?

The reason I am asking is because you can only have 2 HDMI/DVI ports used, and the 3rd connection has to be a pure DP connection OR uses an active adapter.

Although, if it isn't an active adapter I'm not sure how the 3rd screen was working originally.

"To enable more than two displays, or multiple displays from a single output, additional hardware such as DisplayPort-ready monitors or DisplayPort 1.2 MST-enabled hubs may be required." - straight from AMD's website.

This is the adapter I am using http://www.scan.co.uk/products/scan-dvi-display-port-adaptor-dvi-d-dual-link-(female)-to-display-port-(male). I know it works cause I got it off a friend that used it for his Nvidia surround setup and he never had any problems with it.
