Memory: 4 GB DDR3(transcend)
Processor: Intel(R) Core(TM) i5-3470 CPU @ 3.20GHz (4 CPUs)

NEED URGENT HELP

Solution
Please do not post in all caps. Thanks.
If every other part in your system has been changed, and additional cooling is not helping your graphics card, then it probably is defective. Contact Sapphire about warranty replacement.
It is the monitor, not the PC, which enters power-saving mode when it no longer detects a signal. If it happens when your GPU temp hits only 68C, something is off, because a GPU ought to be able to take much more than that. There may be another component overheating, like VRMs on the board, that are causing the loss of signal.

VRM = voltage regulator module. They provide the power to your CPU (on the motherboard) or GPU (on the graphics card).
Make sure they have enough airflow over them to stay cool. You can use a program like MSI Afterburner (a free download) to control the fan speed on your graphics card. Try setting it higher and see if that helps.

I have a 7750 in my secondary pc and this sleep mode power saving thing happens to me as well sometimes though not while gaming. Its random.I have reason to believe its an AMD graphics drivers issue rather than anything else.

I want to tell you that when i use sapphire trixx to increase my fan speed then my pc takes some more time to go into sleep, and when i am not using sapphire trix ,it takes only 5 minutes to go into sleep,Please help

If memtest reports errors, you have bad or unstable RAM, but you might be able to make it stable by slightly increasing its voltage (as long as you keep it under 1.6V) or slowing it down. The standard for DDR3 RAM is 1.5V; some [cheap] RAM may actually need more than that to reach its advertised timings. The Intel IMC is stressed by excessive voltage, so be careful.
If the command rate has been set to 1T, change it to 2T.
Do you have good airflow through your case? A front intake fan and a rear exhaust fan should create sufficient flow to keep a HD7750 cool enough.

yes my CM n200 advance is having 120 mm front and rear fan so i think ,i am having good airflow.
i want to know that with memtest you mentioned is memtest86+ or only memetest.

Well, if using Trixx to increase fan speed delays the crashes, then either the temperature sensor is off, or it isn't measuring what is overheating. If you take the side of your case off and aim a common household fan into your PC, what happens?
I think memtest is actually memtest86+.

IDELA TEMP IS 45 C SO I DONT THINK THAT CPU OVERHEATING IS THE ISSUE,ALSO THERE IS NO DELAY WHEN I AM AIMING MY FAN INTO CABINET.SO GIVE ANOTHER SOLUTION.HELP ME PLEASE
