Buying new pieces, need some help, please.

So over the past couple weeks, my computer has been crashing because of my graphics, I've done many tests and googled my problem and my windows 7 gave me a message when I crashed and it said my display driver has stopped working, lol. But it's not just on WoW, but many other games too, like LoL, Heroes of the storm alpha, Sc2, Diablo 3 RoS, and Warcraft3 and rollercoaster tycoon, so it's not just new games, and I play on good and low settings which is what games recommend me for right now.

The problem
I used to crash about once or twice a day and then it started becoming worse and crashing multiple times per day and then every couple of hours and now it refuses to work for more than 20 minutes. It also crashes even if I'm just on the internet.

What happens
My screen freezes and I get all these little squares and they're different colors most of the time, like pink, and green mostly and sometimes my screen turns black and my monitor light just blinks but won't showing anything and other times I get the blue screen of death and my computer has to restart.

My current graphics card is about 6 years old. It's a Nvidia geforce gts 250
I plan on buying a nvidia geforce gtx 760, but I don't know if my computer can handle it in it's current state. I'm trying to stay between 450-600 bucks. How can I link the other specs of my computer so you guys can see what else I have to buy along with the graphics card, like my processor, motherboard, ram, cpu, memory, hard drive and something else I think?

You cant just link it to the forums, just type it.
If you want to see the info because you do not know then download CPUZ made by CPUID.

Most likely you will need a better power supply, dont know if you need anything else yet or not.

edited: If this is a facotry OEM computer (made by dell, hp, etc) will need to know model number, cpu, ram, and motherboard model. If it is a custom built computer will need to know cpu, motherboard, ram, power supply (brand and capacity), case and hard drive model would also be usefull. The cpu,ram,mobo you can get from cpuz but the psu and hard drive info would require you to look inside the case.
