Coolermaster Hyper EVO 212, its cheap, quiet and used by most of the gamers out there.

This Cooler is a tower and its not small, make sure you have enough space inside your case.

I have a Define R4 case and would a 412S fit?

If I get the 212 what extra fan would you recommend?

You can have mine for £15 if you want (I upgraded to a NH-D15). It was inside my R4 no problems.

There's virtually no benefit of a second fan btw but the mounting kit is there.
